Proliferative diabetic retinopathy: what the trials found

Several anti-VEGF agents have been evaluated for the management of proliferative diabetic retinopathy (PDR). Brolucizumab 6 mg demonstrated significant efficacy in maintaining vision and reducing PDR, with a statistically significant number of subjects showing no PDR at week 54 (p<0.001) and week 96 5. Ranibizumab has shown varying results across studies; while one trial showed a significant reduction in central subfield thickness via OCT (p<0.01) 7, another reported significant improvements in the ability to complete panretinal photocoagulation (PRP) without vitrectomy (p=0.05) 6.

Bevacizumab was associated with significantly lower rates of reoperation (p=0.033), intraoperative hemorrhage (p=0.025), and postoperative vitreous hemorrhage (p=0.006) in specific clinical contexts 2. Additionally, bevacizumab showed significant results regarding recurrent vitreous hemorrhage incidence (p<0.05) 13. Other anti-VEGF agents include Conbercept 1, 0.5-mg Ranibizumab 3, and 2-mg Aflibercept 4, the latter of which was evaluated using ETDRS visual acuity letter scores and Snellen equivalent ranges.

Ranibizumab has also been shown to impact neovascularization area (measured in pixels) and macular edema thickness, with some patients achieving a gain of 3 or more lines of vision on ETDRS charts 11. Safety profiles for ranibizumab included specific observations regarding drug-related ocular and systemic risks (p=0.01) 6.
