Chronic myelomonocytic leukemia: what the trials found

Fludarabine has been evaluated in multiple clinical trials for the treatment of chronic myelomonocytic leukemia (CMML) [1, 10, 20]. In one study involving a cohort with varying tacrolimus prophylaxis durations, 33% to 42% of participants were able to discontinue prophylactic tacrolimus, with only one instance of Grade III-IV acute GVHD reported in the 90-day cohort 20.

Azacitidine has been investigated across several trials for CMML [12, 14, 19]. In a large Phase 3 trial (N=454), Azacitidine did not show statistically significant differences in Event-Free Survival (p=0.557), Overall Survival (p=0.152), or Time to AML Transformation (p=0.562) compared to the control group 3.

Guadecitabine was evaluated in a Phase 3 trial (N=417); while it did not show statistically significant differences in Overall Survival (p=0.6063), results were reported for transfusion independence and marrow complete response rates 6. Other established interventions include Glasdegib 5 and ASTX727, the latter of which was evaluated regarding LINE-1 demethylation percentages and Grade 3 or higher treatment-emergent adverse events in MDS/CMML patients 4.

A supersaturated calcium phosphate rinse was shown to maintain a consistent duration of severe oral mucositis (WHO Grade 3 or 4) at 4.5 for the study population 7.
