Chronic myelomonocytic leukemia: what the trials found

Fludarabine has been evaluated in multiple clinical trials for the treatment of chronic myelomonocytic leukemia (CMML) [1, 10, 20]. In one study involving a cohort with varying tacrolimus prophylaxis durations, 33% to 42% of participants were able to discontinue prophylactic tacrolimus, with only one instance of Grade III-IV acute GVHD reported in the 90-day cohort 20.

Azacitidine has been investigated across several trials for CMML [12, 14, 19]. In a large Phase 3 trial (N=454), Azacitidine did not show statistically significant differences in Event-Free Survival (p=0.557), Overall Survival (p=0.152), or Time to AML Transformation (p=0.562) compared to the control group 3.

Guadecitabine was evaluated in a Phase 3 trial (N=417); while it did not show statistically significant differences in Overall Survival (p=0.6063), results were reported for transfusion independence and marrow complete response rates 6. Other established interventions include Glasdegib 5 and ASTX727, the latter of which was evaluated regarding LINE-1 demethylation percentages and Grade 3 or higher treatment-emergent adverse events in MDS/CMML patients 4.

A supersaturated calcium phosphate rinse was shown to maintain a consistent duration of severe oral mucositis (WHO Grade 3 or 4) at 4.5 for the study population 7.

Recent results — preliminary, needs further review

  • DEC-205/NY-ESO-1 Fusion Protein CDX-1401 was evaluated in a Phase 1 trial (N=8) 2.
  • Fludarabine phosphate was investigated in a Phase 2 trial (N=62) 9.
  • Donor Lymphocytes were evaluated in a Phase 2 trial (N=55) 8.

For the clinician treating this condition

  • Fludarabine is established as an intervention for CMML, with data available regarding its use alongside tacrolimus prophylaxis [1, 10, 20].
  • Azacitidine has been tested in large-scale trials; however, significant improvements in EFS, OS, or time to AML transformation were not observed in the primary Phase 3 study reported [3, 12, 14, 19].
  • Guadecitabine showed no statistically significant difference in Overall Survival compared to control groups in a large trial 6.

Questions about Chronic myelomonocytic leukemia

Can ruxolitinib help CMML patients who also have connective tissue disease?

A specific case report shows that combining azacytidine with ruxolitinib helped a patient with connective tissue disease, CMML, and myelofibrosis by reducing fever, correcting high monocyte counts, and improving anemia.

What happens if a CMML patient is first diagnosed with ITP?

CMML can be misdiagnosed as ITP when thrombocytopenia is the first symptom; immunosuppressive therapy may temporarily raise platelets, but the underlying CMML progresses. Decitabine can stabilize the condition.
