What the trials found For clinicians

Glioblastoma multiforme: what the trials found

In a phase 3 trial, imatinib mesylate showed limited efficacy in glioblastoma multiforme, with progression-free survival rates of 5.3%, 6.6%, 2.1%, and 2.1% during the study duration 1.

A phase 2 trial of temozolomide reported grade 3 or 4 lymphopenia (2 patients), thrombocytopenia (0), neutropenia (1), leukopenia (0), and anemia (0 in one group; 1 in another) 2. Quality of life changes were also assessed, with physical well-being scores ranging from 4.00 to 8.32 and social/family well-being scores from 22.45 to 24.29 2.

Recent results — preliminary, needs further review

  • A phase 2 trial of pp65-shLAMP DC with GM-CSF reported no significant improvement in overall survival (16.9 vs 16.2 vs 20.6 months, p=0.196) or progression-free survival (6.1 vs 6.4 vs 7.1 months, p=0.354), but a significant difference in ELISPOT assay (126.3 vs 48.4 vs -162.7, p=0.038) 4.
  • A phase 2 trial of valproic acid showed improved 1-year event-free survival (12 vs 24 events, p<0.05) but grade 3 thrombocytopenia (1%) and neutropenia (3%) were noted 18.
  • A phase 2 trial of unpulsed DCs reported median overall survival of 16, 20, and 19 months (p=0.072) and a significant difference in dendritic cell migration to lymph nodes (6.0% vs 9%, p=0.0195) 16.
  • A phase 2 trial of SL-701 with poly-ICLC reported overall survival at 12 months (20 vs 14 patients) and progression-free survival at 6 and 12 months (1.9 vs 5.6 months) 10.
  • A phase 2 trial of topotecan reported progression-free survival at 6 months for bevacizumab-naive patients (0.44) and at 3 months for prior bevacizumab patients (0.18, 0.11, 0) 21.

For the clinician treating this condition

  • Imatinib mesylate demonstrated limited efficacy in glioblastoma, with low progression-free survival rates 1.
  • Temozolomide was associated with manageable hematologic toxicities and variable quality of life outcomes 2.
  • Several phase 2 trials have explored novel agents, but none have yet shown definitive survival benefits in established settings.
