Hyperparathyroidism: what the trials found

Cinacalcet has demonstrated significant efficacy in reducing parathyroid hormone (PTH) levels and managing mineral metabolism. Trials showed a 30% reduction in mean PTH at both 6 months 3 and 12 months 3, with specific cohorts achieving PTH targets of $\leq$ 300 pg/mL [1, 3]. Additionally, Cinacalcet was associated with significant reductions in serum phosphorus 14 and absolute decreases in PTH values 8. Clinical outcomes included a high percentage of patients achieving corrected total serum calcium below 10.2 mg/dL 14, while some data indicated improvements in Ca x P values 7 and specific targets for calcium and phosphorus levels 7.

Etelcalcetide demonstrated significant reductions in parathyroid hormone across multiple studies, with high percentages of patients achieving a $\geq$ 30% reduction from baseline [10, 20, 21] and a $\geq$ 50% reduction [10, 12, 21]. In pre-dialysis populations, Etelcalcetide showed superior results in reducing mean PTH compared to other cohorts [12, 20].
