Fludarabine is established in the treatment of myelofibrosis [1, 19]. INC424 demonstrated a 71.7% rate of participants achieving at least a 50% reduction in spleen length over 5 years 2. Ruxolitinib showed significant efficacy in reducing spleen volume, with 31.9% of patients achieving a $\geq$35% reduction by week 24 and 28.5% by week 48 8; it was also evaluated in smaller cohorts 12.

Momelotinib demonstrated superior outcomes compared to placebo across multiple metrics: a significantly higher splenic response rate (SRR) of $\geq$25% at week 24 (39.23% vs. 6.15%, p<0.0001) 4, a significant reduction in the number of red blood cell (RBC) transfusions per month (p<0.001) 3, and a significantly higher rate of transfusion independence at week 24 (30.0% vs. 20.00%, p=0.0116) 4.
