Urinary Incontinence: what the trials found

Pharmacological and procedural interventions for urinary incontinence show varying degrees of efficacy. OnabotulinumtoxinA treatment is associated with significant reductions in the daily average number of urinary incontinence episodes 2 and a reduction in micturition episodes 2. Cystoscopic injection of Botox into the bladder also demonstrated a statistically significant reduction in daily incontinence episodes 5. Furthermore, multiple trials involving OnabotulinumtoxinA reported high percentages of patients achieving substantial reductions (≥50%, ≥75%, or ≥90%) in daytime urinary incontinence episodes across multiple treatment cycles 16, and some studies noted a 100% reduction in episodes for a portion of the cohort 19.

Surgical interventions show measurable outcomes in success rates. Retropubic mid-urethral sling (TVT) resulted in objective treatment success rates of approximately 77-81% at 12 months 8. The Burch Modified Tanagho procedure showed a statistically significant difference in 24-month cumulative stress-specific success rates between study arms 22. Grafted anterior prolapse repair was associated with lower rates of recurrent Stage II or greater anterior vaginal prolapse compared to other methods 9.

Behavioral and supportive interventions provide evidence for symptom management. Behavioral therapy resulted in a significant number of patients reporting 'Much Better' or 'Very Much Better' on the Patient Global Impression of Improvement (PGI-I) at both 3 and 12 months, with a corresponding reduction in bothersome stress incontinence symptoms 11. Office evaluation protocols were associated with high rates of patients meeting or exceeding a 70% decrease in UDI scores and improved patient global impressions 7.
