Vitiligo: what the trials found

Ruxolitinib has demonstrated significant efficacy in improving facial vitiligo scores. In a Phase 3 trial, patients receiving ruxolitinib showed high rates of improvement, with approximately 98.2% achieving a ≥50% improvement in F-VASI scores and 96.4% achieving a ≥75% improvement during the extension period 1.

Ruxolitinib cream was shown to significantly improve facial vitiligo compared to vehicle at week 24, where 45.5% of patients achieved a ≥50% improvement in F-VASI scores (p=0.0057) 6. However, significant improvements in full-body assessment (T-VASI) were not statistically confirmed at week 52 6.

Ruxolitinib cream treatment was associated with a reduction in the immune biomarker CXCL10 at week 4 and week 24, with these changes correlating significantly to efficacy readouts (p=0.004) 5.

Recent results — preliminary, needs further review

  • Tofacitinib showed significant improvements in whole blood transcriptome interferon (IFN) scores (p=0.00000621), Investigator's Global Assessment (IGA) (p=0.00000061), and Dermatology Life Quality Index (DLQI) (p=0.00000004) 2.
  • AMG 714 did not show statistically significant improvements in F-VASI35 or F-VASI25 scores at various time points through week 48 3.

For the clinician treating this condition

  • Ruxolitinib is established as an effective treatment for facial vitiligo, with high percentages of patients achieving significant improvements in F-VASI scores [1, 6].
  • Ruxolitinib cream demonstrates a correlation between the reduction of inflammatory biomarkers (CXCL10) and clinical efficacy 5.
  • Tofacitinib shows promise in improving both biological markers (IFN score) and patient-reported quality of life (DLQI) 2.
