Partial-Onset Seizures: what the trials found

Lacosamide has demonstrated significant efficacy in reducing partial-onset seizure frequency. In a study of 548 patients, Lacosamide 50 mg resulted in a statistically significant reduction in seizure frequency per 28 days compared to baseline (p<0.001) 8. Another trial showed a 44.47% reduction in partial-onset seizure frequency per 28 days from baseline 3. Additionally, Lacosamide was associated with high rates of patients remaining on treatment without discontinuation due to adverse events or lack of efficacy (81.3%) over periods up to 12 months 6.

Perampanel has shown varying results in clinical trials for partial seizures. One study reported a reduction in seizure frequency per 28 days, though the result did not reach statistical significance (p=0.2330) 2. In another trial, 63% of patients on 4 mg Perampanel achieved seizure-free status during a 26-week maintenance period, while 74% achieved this at the last evaluated dose of 4 or 8 mg; however, only 32.9% remained seizure-free after a 52-week treatment phase 4. Retention rates for Perampanel were reported at 85.2% at 3 months, 68.5% at 6 months, and 63.0% at 9 months post-initiation 1.

Other interventions include Levetiracetam (LEV) and Pregabalin. For LEV, approximately 73.8% of patients in the 1000 mg/day to 2000 mg/day group were seizure-free for 26 weeks, falling to 59.0% at 52 weeks; notably, only 22.2% were seizure-free in the 3000 mg/day group 9. Pregabalin showed a responder rate (≥50% reduction in 24-hour seizure rate) of 30.51%, 53.57%, and 41.51% across three dose levels, but these results were not statistically significant (p=0.2418) 5. TRI476 showed a response rate of 23.4% in the treatment group compared to 3.9% in the comparison group 10.
