What the trials found Updated — new results For clinicians

Alzheimer's Disease: what the trials found

Donepezil has been evaluated across multiple studies for its impact on cognitive and functional outcomes. Trials showed changes in MMSE scores 16, ADAS-Cog scores [17, 21], and activities of daily living (ADCS-ADL) 16. Additionally, Donepezil was associated with specific changes in the Selective Reminding Test (SRT) 18 and clinical global impression (CGI) scores 19. One study noted a significant increase in the Hippocampus Connectivity Index (HCI) over 12 weeks 21, while another showed no significant correlation between APOE genotype and the incidence of adverse events for Donepezil 16.

Rivastigmine was shown to result in a change from baseline in ADCS-ADL SIV scores 20. In a separate study, Ramipril was associated with a significant decrease in cerebrospinal fluid (CSF) Angiotensin Converting Enzyme (ACE) levels compared to placebo over 4 months 22, though it did not significantly impact CSF Amyloid Beta-42 levels or flow-mediated vasodilation 22.

Propofol and other anesthetic agents were evaluated for their impact on cognitive indices; however, no significant differences were found in the change of CSF markers of Alzheimer's disease between anesthetic groups 15.

Recent results — preliminary, needs further review

  • Lecanemab (2.5 mg/kg) showed changes in ADCOMS scores at 12 months and significant reductions in brain amyloid pathophysiology as measured by PET at 12 and 18 months 3.
  • Belly-focus meditation was associated with a statistically significant change in plasma Amyloid Beta (Aβ) levels 14, though it did not significantly impact the Ab42/40 ratio or pTau-181/Tau ratio 14.
  • Brain training showed no significant changes in plasma Aβ levels, Ab42/40 ratios, or brain perivascular space volume 13.
  • The MEMORI Corps program was associated with a change in Neuropsychiatric Inventory (NPI) scores 10.

For the clinician treating this condition

  • Donepezil is established as an intervention affecting multiple cognitive markers including MMSE, ADAS-Cog, and functional activities of daily living [16, 17, 21].
  • Rivastigmine has been shown to impact activities of daily living scores in patients with severe impairment 20.
  • While some non-pharmacological interventions like belly-focus meditation show significant changes in plasma Amyloid Beta levels, other methods such as brain training did not produce statistically significant changes in similar biomarkers [13, 14].

Questions about Alzheimer's Disease

What is the link between Periodontal Disease and Alzheimer's disease risk?

Periodontal disease is linked to a higher risk of Alzheimer's disease, likely through inflammation and oral bacteria reaching the brain, but a direct cause is not proven.

Full answer →
Is there a genetic link between kidney function and Alzheimer Disease for my family?

Current research finds no overall genetic link between kidney function and Alzheimer disease, though some shared biological pathways may exist.

Full answer →
Does having brain reserve protect me from Alzheimer Disease symptoms if I am inactive?

Research shows that having more brain reserve, such as from higher education, can help maintain cognitive function even when Alzheimer's disease pathology is present, even in people who are physically inactive.

Are there new genetic markers for Alzheimer disease found in veteran populations?

Yes, a meta-analysis of the VA Million Veterans Program identified 17 new genetic loci in European, 4 in African, and 3 in Hispanic veteran populations for Alzheimer's disease.

Full answer →
Do genetic tests find new risk factors for Alzheimer's in veteran populations?

Yes, genetic tests can find new risk factors for Alzheimer's in veteran populations, as shown by a meta-analysis including the VA Million Veterans Program that identified 17 new genetic loci in European ancestry veterans.

What is the new FDA-approved medication for agitation in Alzheimer's disease?

The FDA has approved Auvelity (dextromethorphan/bupropion) for treating agitation in Alzheimer's disease, making it the first medication specifically approved for this indication.

Full answer →
Can exercise programs really improve sleep quality for people with Alzheimer's disease?

Yes, exercise programs can improve subjective sleep quality in people with Alzheimer's disease, based on a meta-analysis of 12 randomized trials showing a significant reduction in PSQI scores.
