Seasonal allergic rhinitis: what the trials found

For seasonal allergic rhinitis, several treatments have demonstrated efficacy. A fixed combination of azelastine hydrochloride and fluticasone propionate nasal spray (Dymista) significantly improved total nasal symptom scores (TNSS) and total ocular symptom scores (TOSS) compared to placebo 2. Similarly, another azelastine/fluticasone combination showed significant improvements in reflective and instantaneous TNSS 22. Fluticasone furoate combined with fexofenadine significantly reduced nighttime symptom scores and reflective total nasal symptom scores 717. Olopatadine HCl nasal spray (0.6%) improved reflective and instantaneous TNSS and reflective TOSS 1114. Levocetirizine dihydrochloride significantly reduced the 24-hour reflective total 5 symptoms score and improved rhinoconjunctivitis quality of life in one trial 15, but another trial showed no significant difference 16, indicating mixed evidence. Mometasone furoate nasal spray did not show significant improvement in TNSS at 6 hours post-dose 12 or in total asthma symptom severity 4, but did significantly improve cough symptom scores 4. QNASL did not significantly improve TNSS or RQLQ scores 3. Vitamin D3 supplementation significantly improved daytime nasal symptom scores but not total nasal symptom scores 9. PL nasal spray significantly improved total eye symptoms, total nasal symptoms, and sneezing 10.

Allergen immunotherapy also shows benefit. PQ Grass significantly improved the combined symptom and medication score (CSMS) during peak grass pollen season in two trials 120. Short ragweed pollen allergen extract significantly improved total combined score and rhinoconjunctivitis daily symptom score during peak ragweed season 18. However, MK-3641 (short ragweed extract) was associated with local swelling and application site reactions, leading to discontinuation in some participants 8. Fexofenadine alone was evaluated but only reported adverse events without efficacy outcomes 21. Placebo was used as a comparator in some trials 5.

Recent results — preliminary, needs further review

  • PQ Grass (a grass pollen allergen extract) showed significant improvement in CSMS during peak grass pollen season in two phase 3 trials 120, but one trial also reported non-significant results for CSMS averaged over the entire season 1.
