Gastric adenocarcinoma: what the trials found

In patients with gastric adenocarcinoma, pembrolizumab combination therapy demonstrated statistically significant improvements in Progression-Free Survival (PFS) and Overall Survival (OS) for participants with a PD-L1 CPS ≥1 1. However, OS did not reach statistical significance in the subgroup of patients with a PD-L1 CPS ≥10 1. In another trial involving pembrolizumab in PD-L1 positive participants, while OS showed a statistically significant improvement over the control 2, the PFS did not reach statistical significance 2.

Ramucirumab demonstrated superior Progression-Free Survival (PFS) compared to standard of care 3 and achieved an Objective Response Rate (ORR) of 26.5% vs 21.9% in a Phase 3 trial 3. In a separate Phase 2 study, Ramucirumab showed ORRs of 27.6% and 25.4% across different dosage arms with reported PFS values of approximately 5.16 to 5.42 months 21.

Andecaliximab was evaluated in two trials; one Phase 3 trial showed a statistically significant improvement in Objective Response Rate (ORR) but did not reach statistical significance for Overall Survival (OS) or Progression-Free Survival (PFS) 6. A separate Phase 2 trial of Andecaliximab did not show statistically significant differences in ORR, PFS, or OS compared to the control 19.

Other investigated agents include tucatinib, which showed a mean maximum percentage change from baseline in weight of 0.34% vs 6.21% 4, and Rivoceranib, which was associated with specific changes in quality of life scores (EORTC QLQ-STO22) and visual analog scales (EQ-5D-5L) 5. Cisplatin was evaluated in multiple trials [7, 9].
