Iron Deficiency Anemia: what the trials found

Several iron-based interventions demonstrated significant improvements in hemoglobin (Hb) levels for patients with anemia. Elemental iron showed a statistically significant increase in Hb concentration over time 1. Ferumoxytol was associated with significant increases in Hb concentrations at various time points [14, 15, 17] and showed a median Hb increase of 11.45 units post-intervention 20. Additionally, ferric maltol demonstrated a statistically significant increase in Hb concentration from baseline to week 16 16.

Iron isomaltoside/ferric derisomaltose was associated with significant increases in Hb concentrations at several intervals (weeks 1, 2, 4, and 8) [12, 13]. However, this intervention showed a significantly higher incidence of hypophosphatemia compared to the comparator group across multiple studies [9, 10]. Ferric carboxymaltose resulted in significant increases in both ferritin levels and transferrin saturation (TSAT) from baseline to day 35 22, although the change in Hb concentration was not statistically significant 22.

Other iron preparations showed varying results. Ferrous sulfate was evaluated for changes in ferritin, serum iron, and TSAT at 12 weeks 18. Ferric citrate did not show a statistically significant difference in the composite endpoint of initiation of maintenance dialysis or all-cause mortality 19.
