Ketamine has been shown to reduce suicidal ideation. In a phase 4 trial (N=82), ketamine was associated with a significant reduction in Beck Scale for Suicidal Ideation (BSSI) scores compared to placebo (p=0.047) 6. Another trial (N=24) reported a change in BSSI from 8.8 to 15.3 (p=0.047) 6. Intranasal ketamine was also studied (N=33) and assessed for manic symptoms and dissociative state, but no direct comparison for suicidal ideation was provided 15. Additional trials (N=16) further support ketamine's use 17.
Estradiol transdermal patch (0.1 mg/24 hrs) has demonstrated efficacy in reducing perimenstrual suicidal ideation. In a trial (N=30), perimenstrual change in Suicidal Ideation Score (SITBI) was significantly reduced (p=.012) 8. Another trial (N=124) showed significant reduction in perimenstrual Daily Adult Suicidal Ideation Questionnaire (ASIQ) scores (p<.001) and Columbia Suicide Severity Rating Scale (C-SSRS) planning item scores (p=.026) 9. A third trial (N=60) found significant cyclical change in suicidal ideation severity (p=.002) with transdermal estradiol plus placebo 11.
Other interventions have been studied with mixed or limited evidence. Paliperidone (N=54) showed no significant difference in Beck Scale for Suicidal Ideation scores compared to lithium or placebo after 3 months (mean scores 22.1, 22.1, 21.3) 14. Paroxetine CR (N=15) was associated with a reduction in SSI score from 11 to 0.3 13. Prazosin (N=20) showed a change score of 7.6 to 8.2 on the Scale for Suicide Ideation 12. Zolpidem-CR (N=103) and uridine (N=75) were studied but no specific outcomes for suicidal ideation were reported 710.
