Gastric Cancer: what the trials found
Pembrolizumab has demonstrated significant improvements in overall survival (OS) across multiple cohorts. In one study, OS was significantly improved in all participants (12.9 vs 11.5, p<0.0001), with even greater gains in patients with PD-L1 CPS ≥10 (15.7 vs 11.8, p<0.0001) 6. Another trial showed significant OS benefits for all participants (8.4 vs 5.6, p=0.0015), with specific benefits noted for patients with PD-L1 CPS ≥10 (12.0 vs 5.3, p=0.0008) 19. Additionally, pembrolizumab combined with XP/FP showed a statistically significant improvement in event-free survival (EFS) (44.4% vs 25.7%, p=0.01501) and a significantly higher pathological complete response (pathCR) rate (13.4% vs 2.0%, p<0.00001) 10.
Other immunotherapies and combinations have shown varying results. Nivolumab plus chemotherapy significantly improved both OS (14.39 vs 11.10, p<0.0001) and progression-free survival (PFS) (7.69 vs 6.05, p<0.0001) in patients with PD-L1 CPS ≥5 14. Tislelizumab showed significant improvements in OS for both PD-L1 positive participants (17.2 vs 12.6, p=0.0056) and the intent-to-treat population (15.0 vs 12.9, p=0.0011) 20. TAS-118 plus Oxaliplatin also demonstrated significant improvements in OS (16.0 vs 15.1, p=0.039), PFS (7.1 vs 6.4, p=0.0045), and time to treatment failure (6.1 vs 5.3, p=0.011) 18.
In contrast, some agents did not reach statistical significance in primary endpoints. TAS-102 showed a significant improvement in OS (5.7 vs 3.6, p=0.0003) 7, while Avelumab did not show significant improvements in OS (p=0.1779) 15 or in OS, PFS, or ORR (p>0.05) 8. Other trials, such as those involving 5-Fluorouracil 9, Olaparib 12, and Ramucirumab 13, provided data on response rates and survival but did not reach statistical significance for primary survival endpoints in the reported data.

For the clinician treating this condition
- Pembrolizumab is associated with significant improvements in OS and EFS, with notable benefits in patients with high PD-L1 expression [6, 10, 19].
- Nivolumab plus chemotherapy and Tislelizumab have demonstrated statistically significant improvements in overall survival [14, 20].
- TAS-118 plus Oxaliplatin showed significant improvements in OS, PFS, and time to treatment failure 18.
- Avelumab did not demonstrate statistically significant improvements in OS or other primary endpoints in the reported trials [8, 15].
