Atopic Dermatitis: what the trials found

Ruxolitinib Cream demonstrated significant efficacy for atopic dermatitis, with 70.0% of participants achieving an EASI75 at week 8 (p=<0.0001) and 61.3% achieving IGA-TS (p=<0.0001) 1. Upadacitinib showed high rates of improvement by week 24, with 88.5% to 97.2% of participants achieving EASI75 and up to 33.8% achieving EASI100 7.

Dupilumab treatment was associated with significant reductions in pruritus (p=<0.001), improvements in sleep quality (p=<0.001), and decreases in SCORAD scores (p=<0.001) 12. Additionally, Dupilumab showed a reduction in peak pruritus scores and EASI total scores over 17 to 21 weeks 11, while also demonstrating significant reductions in transepidermal water loss (TEWL) on lesional skin [10, 13].

Other established treatments include Crisaborole, which was associated with reduced SCORAD scores and pruritus scores 8, and various topical agents such as triamcinolone cream 15, desoximetasone spray 16, fluocinide (which improved EASI and TEWL) 17, tacrolimus ointment 18, desonide hydrogel 19, and hydrocortisone 17-butyrate 20.

Additional established interventions include Atopiclair Nonsteroidal Cream 4, Omalizumab 14, and Montelukast 22.

Recent results — preliminary, needs further review

  • LEO 138559 showed significant mean percentage reductions in EASI scores across multiple assessment points (p=0.0090) 2.
  • Placebo data indicated that while some improvements were noted, the results did not reach statistical significance for EASI75 or vIGA 'clear' status 3.

For the clinician treating this condition

  • Ruxolitinib Cream and Upadacitinib are established options with high rates of EASI75 achievement.
  • Dupilumab provides multi-modal benefits including significant reductions in pruritus, improved sleep quality, and reduced transepidermal water loss.
  • Various topical agents (e.g., Crisaborole, tacrolimus, desonide) are established for managing eczema severity and itching.
