Psoriasis: what the trials found

Secukinumab showed no significant difference in PASI 90 at week 28 compared to placebo (p=0.3857) but demonstrated a significant difference in PASI 75 over time (p=0.0300) 1. Risankizumab significantly improved PASI 90 and sPGA 0/1 at week 16 versus placebo (both p<0.001) 7. Deucravacitinib significantly improved DLQI 0/1, DLQI ≥4 reduction, and itch NRS at week 16 (p=0.0001 to p<0.0001) 14, and significantly improved scalp-specific outcomes (Ss-PGA 0/1, PSSI 90, scalp itch NRS) at week 16 (all p<0.0001) 16. Apremilast significantly improved DLQI ≥4 reduction and DLQI change at week 16 (p<0.0001) and BSA percent change (p=0.0085) 11. Tildrakizumab significantly improved quality of life (PGWB and DLQI) over time (p<0.001 to p=0.011) 10. Methotrexate showed significant improvement in biologic activity endpoints (p<0.05) and clinical endpoints (PASI 75: 67% vs 27%; PGA 0-1: 73% vs 27%) 3. Efalizumab resulted in 68% of patients achieving PGA good or better, with 11% rebound and 12.9% exacerbation 8; another trial reported 46% achieving PGA excellent or cleared at week 24 9. Tapinarof cream 1% led to 23% of patients achieving target lesion PGA clear/almost clear in head/neck 17 and 24% achieving intertriginous PGA clear/almost clear 18. Adalimumab showed no significant change in vascular inflammation or cardiometabolic biomarkers (p>0.05) 4. Apremilast showed no significant change in aortic vascular inflammation (p=0.612) or cardiometabolic markers (CRP p=0.617, ferritin p=0.083) 6. Tildrakizumab showed minimal change in coronary flow reserve (-0.03) and no significant correlation with skin severity (p=0.35) 13. Metaderm improved sPGA*BSA (53.06% vs 65.6%) and DLQI (52.88% vs 17.82%) 21. Apremilast improved nail psoriasis (mNAPSI mean percent change 64.1% at week 36, p=0.05) and nail pain VAS (-50.42%) 22. Secukinumab improved itch, pain, and PASI scores (change: 3, 2, 8 respectively) 15.
