Psychotic disorders: what the trials found

Pharmacological management of psychotic disorders includes several established agents. Risperidone and its long-acting injection (LAI) have demonstrated significant efficacy in reducing PANSS total scores [7, 17], improving personal and social performance 8, and significantly reducing relapse rates at weeks 32 and 48 9. Risperidone LAI was also associated with a reduction in the frequency of drinking days 10.

Other established antipsychotics include Paliperidone ER, which showed significant reductions in PANSS total scores and higher response rates compared to other treatments 19, as well as improvements in personal and social performance 8. Aripiprazole has been shown to significantly reduce PANSS total scores and improve Clinical Global Impression (CGI) scores 17 while showing changes in cognitive measures across different study designs 15.

Additional established treatments include Olanzapine, which showed a significant reduction in the Scale for the Assessment of Negative Symptoms 12, and Pimozide, which demonstrated reductions in both positive and negative syndrome scales 6. Quetiapine was evaluated regarding drinking habits 13, while Clozapine was studied in relation to cannabis use 11.

Recent results — preliminary, needs further review

  • iTEST showed a significant improvement in trained introspective accuracy (p<0.001) and a statistically significant change in the Specific Level of Function Scale (p=0.05) 4.
  • The Veteran Voices and Visions intervention did not produce statistically significant changes in psychotic symptoms, role functioning, or beliefs about voices 3.
  • Newer interventions including A4i 1 and DC Stimulator PLUS 2 are currently under investigation.
