Hip Fracture: what the trials found

For pain management after hip fracture surgery, local anesthetics such as Exparel 4 and levobupivacaine 5 have been studied, as well as intravenous fentanyl 6 and intravenous morphine 8. A phase 4 trial of IV morphine showed significant pain reduction at 30 minutes (p<.0001) and 60 minutes (p<.05) compared to comparators 8. Another trial reported a pain score change of 4 vs 2 with IV fentanyl 6.

Tranexamic acid (TXA) has been evaluated in multiple trials for reducing transfusion needs. One phase 4 trial found a lower mean number of units transfused (1.8 vs 1.2) 7, while a phase 3 trial showed no significant difference in transfusion rates (p=0.966) 12. Mortality was not significantly different (p=0.75) 7. The evidence on TXA's effect on transfusion is mixed.

Testosterone therapy was assessed in two phase 3 trials. One trial found a significant increase in total lean body mass (p=0.01) but no significant change in muscle strength 13.
