Ventilator-associated pneumonia: what the trials found

Pharmacological management of ventilator-associated pneumonia includes several agents with documented clinical outcomes. Zavicefta (Ceftazidime-Avibactam) demonstrated a clinical cure rate of 59.3% in the mMITT population and 66.7% at end-of-treatment compared to 25.9% and 29.6%, respectively, in the comparator group 2. ATM-AVI showed clinical cure rates of 68.4% (ITT) and 72.9% (Micro-ITT) compared to 65.7% and 72.3% in its respective comparison groups 6.

Ceftolozane/tazobactam was evaluated for mortality, showing a 24.0% all-cause mortality rate at day 28 in the ITT population compared to 25.3% in the comparator group 5. Clinical response and cure rates for this intervention were approximately equal between groups (54.4% vs 53.3%) 5.

Doripenem was evaluated in multiple trials; clinical response at end-of-treatment varied across study arms, with results ranging from 20.8% to 71.2% 3. Clinical outcomes for doripenem also included data on APACHE II scores and rates of ICU or hospital readmission within 28 days post-treatment 4.

Infection control measures include the use of chlorhexidine gluconate, which was associated with a significantly lower incidence of nosocomial infections (15% vs 30%, p=0.049) and similar ICU lengths of stay [17, 18]. Additionally, ETT cleaning maneuvers were established as an intervention 13, and high-definition CT imaging was used to assess occlusion of extubated tubes 14.
