Crohn's Disease: current evidence & treatments

Crohn's disease (CD) involves complex pathophysiological mechanisms including mucus-barrier dysfunction, ER stress, and microbial sensing, as shown by transcriptomic meta-analysis of 1,302 active, untreated human samples 1. Genetic studies have identified 619 independent association signals for inflammatory bowel disease (IBD) across 420 regions, accounting for 77-80% of SNP-based heritability 6. Oral microbiome analysis reveals significantly reduced Shannon diversity in CD patients compared to controls 10. Additionally, IgG glycan profiles can discriminate IBD from controls, with IBD patients showing accelerated biological aging 20. Neutrophil SIRL-1/IL-8 ratio has been evaluated as a diagnostic biomarker for IBD 17.

Several therapeutic options are available for CD. The FDA has approved adalimumab (Hadlima) for pediatric CD (ages 6 and older) 7 and ustekinumab (Stelara) for adult CD 1415. A Phase 3 trial of darvadstrocel for perianal fistulas showed a 48.8% combined remission rate but was not superior to placebo 3. In pediatric CD, exclusive enteral nutrition was superior to corticosteroids for inducing clinical remission (OR 1.72) and mucosal healing 13. Enteral nutrition therapy in a Chinese cohort achieved 62.5% remission in CD over 6 weeks 11. Upadacitinib showed a 94.1% remission rate at 26 weeks in a retrospective CD cohort 18. Guselkumab is under investigation in Phase 2/3 trials for moderately to severely active CD 25. The Mediterranean diet showed no significant advantage over control diets for inducing remission in IBD 4.

Diagnostic approaches include video capsule endoscopy (VCE) in children, which often requires endoscopic placement (25% of examinations) and has diagnostic utility for CD 21. Small bowel capsule endoscopy as initial investigation reduced the need for double-balloon enteroscopy in suspected small bowel CD 23. Risk factors for CD include childhood antibiotic use, which increases IBD risk (RR 1.42) and specifically CD risk (RR 1.59) 8. Cesarean section was not associated with IBD risk in a meta-analysis of over 13 million births 24. Pediatric IBD patients have increased risk of venous thromboembolism (RR 6.94) 22. Brain alterations in CD localize to emotional and interoceptive networks, with distinguishable phenotypes from ulcerative colitis 2.
